Nel 1975 debutta alla Scala di Milano come Riccardo in "Un ballo in maschera" (Giuseppe Verdi). All'età di 28 anni Carreras vanta un repertorio di 24 opere. Raccoglie applausi entusiasti in tutto il mondo, dall'Arena di Verona all'Opera di Roma, dall'Europa al Giappone e nelle due Americhe.
