I would usually say 'follow' for a doctor's advice because doctors don't have that kind of power. I would say 'obey' if there have been official instructions to stay inside.

Obey would mean "To give ear to; to execute the commands of; to yield submission to; to comply with the orders of." In order to obey, one must do something, and needs to be told to do it.
